Title: Social Worker MSW
Location: Maury Regional Center in Columbia, TN
Position Shift: PRN
Position Summary:
The social worker evaluates a patient's social, emotional, environmental, financial, and support needs. Assesses, plans and acts in response to the psychosocial needs of patients/families. Intervenes in crisis situations to assist patients. Facilitates decision making on behalf of patients and families in complex cases. Collaborate with care manager in assessment and coordination of discharge planning. Provides interventions designed to promote health, prevent disease, and address barriers to access to healthcare.
Credentials / Education:
* Social Worker Licensure Required
* Bachelor's Required, MSW Preferred
* 1 Year Social Services Experience in a Hospital Preferred
