**Overview** Founded in 1971, Dialysis Clinic, Inc. (DCI) remains the nation's largest non-profit dialysis provider. With a team of approximately 5,000 talented and caring employees serving across the United States, DCI provides care to nearly 15,000 patients on dialysis and 3,500 patients with chronic kidney disease. We offer a variety of services including in-center hemodialysis, home hemodialysis, and peritoneal dialysis (PD). The **Medical Records Clerk** will perform clerical duties associated with obtaining,completing,andmaintainingpatient medical records requests.Follows established policies and procedures to ensure effective and compliant record management.Manages process end-to-end for requestsof medicalrecords;including accuracy, minimum necessary, and submission standards. Ensures the privacy and security of all patient records. **Responsibilities** The following is a summary of what makes our team successful. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. + Answers and facilitiates requests for medical and billing records frominternal and externalparties. + Responsible for retrieving/collectingmedical records and information from the EMR system. + Review accuracy of records prior to release to external parties. + Handle record invoicing per stateand federallaws. + Maintainaccuratelogs and information release forms for medical recordrequests. + Research lost or missing records/informationin accordance withestablished procedures. + Setup reviewsfor external chart reviews, provide training, and address questions. + Distribute records and reports to departments asrequested. **Qualifications** + High school diploma or equivalent required. + Understanding of healthcare regulationsand federal HIPAA privacy regulationsrequired. + Previousexperience in the handling of patient health information and/or medical records is strongly preferred. + RHIT(Registered Health Information Technician)or RHIA(Registered Health Information Administrator)certification preferred.
